The first step towards a beautiful, healthy smile is to schedule an appointment. Please call the office at your earliest convenience to book a consultation with one of our doctors.

Patient Forms

If you are a new patient to our office, please make sure to create a mychart account to fill out all our forms online ahead of time. You should have received a welcome email or text with a link. If you didn't receive one please call the office. Filling these out ahead of time ensures more time with your doctor which will allow us to attend to your medical needs more quickly and thoroughly than completing them on your arrival. Payment Options

For your convenience, we accept cash, personal checks, debit cards, FSA, HSA and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements. We currently do not offer in house financing. Financing may be done through Care Credit or Sunbit, which offer 0% deferred financing upon approval and qualifying credit score.

Dental Insurance

We accept most dental PPOs. We will provide a personalized treatment plan estimate based on your insurance. Our staff will help prepare insurance forms or assist in making collections from insurance companies and will credit any collections to your account. However, we cannot render services on the assumption that our charges will be paid by an insurance company.
